Hi there, we just purchased a home and are going to be replacing our fireplace surround. The fireplace surround is original to the house (1927), and we have a feeling it was a kit that may pre-date the house. It has 2" yellow tiles that have some crackling in the light lacquer as well as 4" (unfortunately not 6") square tiles in the same red and blackish-blue that you are looking for. Are you interested? Have a look at Walls and Floors website, specifically their Melford design which I think is the same as this (and their Classic border design). You can buy individual tiles from them if you find what you want. Look for the Victorian spares option. I think your colours are red, black and cognac.
